If you misrepresent yourself in any way during the Booking Service process, including but not limited to false information about your age or medical history, Zeus Men’s Health reserves the right to cancel your scheduled service without prior notice and charge you the full fee. Additionally, if you are under the influence of drugs or alcohol at the time of your appointment, the service will not be performed, and you will be billed the full amount.

6. Payments and Subscription Services

You are responsible for paying all fees associated with the services you book through our platform. Payment methods and terms will be provided at the time of booking. Payments are processed through secure third-party payment processors, and we do not store your payment information. Failure to pay any fees may result in the termination of your access to the Service. All fees are non-refundable, except as provided in our Refund Policy.

If you subscribe to any subscription services offered through the Service, you agree to pay the subscription fees and any other charges incurred in connection with your subscription, including any applicable taxes. Subscription services may automatically renew unless you cancel in accordance with the terms of the subscription.

When utilizing the Booking Services, you consent to Zeus Men’s Health automatically billing your credit card or other payment methods for the relevant fees or charges, along with any applicable taxes, for the services reserved via our platform. This authorization will stay effective until you revoke it in line with our cancellation policy.

Refunds are available under specific conditions, such as if a service provider is unable to perform the booked services due to circumstances beyond the user’s control. Refund requests must be submitted within 30 days of the service date. Refunds are processed within 10 business days of approval. We may terminate your access to the Service at any time, with or without cause, and without prior notice. Upon termination, you must cease all use of the Service. Emergency Protocols: In case of a medical emergency during a telehealth session, users should immediately contact local emergency services or go to the nearest emergency room.

Cancellation of Services: A minimum of 24 hours’ notice is required for the cancellation of any scheduled service. If you fail to give at least 24 hours’ notice, you will be billed the full amount of the service booked. For services booked within 24 hours of the appointment, cancellations are not permitted. This policy also applies to group bookings (for two or more individuals); the full amount for all participants will be charged unless a 24-hour advance notice of cancellation is provided.

By using the services of Haad MD Inc DBA Zeus Men’s Health (“Zeus Men’s Health”), you authorize the healthcare providers and their staff to perform any necessary medical diagnostic procedures and treatments to diagnose and/or treat your condition. You have the right to give, withhold, or withdraw consent for any medical treatment at any time.

Scope of Services

Zeus Men’s Health provides services including telemedicine consultations, lab tests, prescription medications such as peptides, weight loss medications, and testosterone replacement therapy. The healthcare providers will explain the proposed treatments and their associated risks and benefits, and you have the opportunity to ask questions before proceeding. Prescriptions are provided by licensed medical professionals based on their professional judgment. You are responsible for providing accurate medical information and for complying with all instructions related to your prescriptions.

Telemedicine Services

  • You consent to receiving healthcare services via telehealth/telemedicine methods and acknowledge the following risks:

  • Potential breaches of privacy due to internet transmission.
  • Possible technical issues that may disrupt consultations.
  • Risk of miscommunication during telehealth sessions.
  • Telehealth/telemedicine may not always be as effective as in-person consultations.
  • It is your responsibility to use a private and secure location for telehealth consultations.

2. Financial Responsibility Guarantee

You accept full financial responsibility for all fees associated with services provided by Zeus Men’s Health, including telemedicine consultations, lab tests, and prescribed medications.

Insurance and Payment

You are responsible for providing accurate and current insurance information. You authorize Zeus Men’s Health to submit claims to your insurance carrier for services provided. If your insurance does not cover certain services or medications, or if you do not have insurance, you are responsible for full payment.

Payment Methods

You agree to provide valid credit card information for billing purposes and authorize Zeus Men’s Health to charge your card for any services rendered. Payment is due at the time services are provided unless other arrangements have been made in advance.

If your account becomes delinquent, your access to services may be restricted until your account is current. Non-payment may result in the use of a collection agency, and you will be responsible for any additional costs incurred.

3. Dispute Resolution

Mediation

Before pursuing any disputes in arbitration or otherwise, both parties agree to voluntarily submit all disputes (except for injunctive relief) to non-binding mediation with a jointly selected neutral mediator. Mediation will take place in Los Angeles, California, within sixty (60) days of written notice of the dispute. The mediator’s fee will be shared equally by both parties.

Arbitration

All disputes not resolved by mediation will be settled by final and binding arbitration, except for disputes prohibited by law from being arbitrated. Arbitration will be conducted by a single neutral arbitrator in Los Angeles, California, under the JAMS Streamlined Arbitration Rules. The arbitrator’s decision will be final and binding, with limited rights of judicial review as provided by law.

4. Privacy and Data Security

We are committed to protecting your privacy and securing your personal information. Please refer to our Privacy Policy for detailed information on how we handle and protect your data.

5. Cancellation Policy

If you need to cancel an appointment, please notify us at least 24 hours in advance. Late cancellations or no-shows may incur a cancellation fee.

All products offered at Zeus Men’s Health require an online consultation with a healthcare provider who will determine if a prescription, if appropriate and safe for the patient. If approved, a prescription will be sent for fulfillment to one of our partner compounding pharmacies by our medical provider. Most orders are fulfilled within 5 business days, but occasionally can be delayed due to backordered medications, compounding delays, medication shortages, or pharmacy shipment delays. Zeus Men’s Health is not responsible for delays in shipment or fulfillment delays due to partner compounding pharmacies as this is out of our control. The medication will be shipped directly to the patients home address provided on the order form once approved, prescribed, and fulfilled. Please note that compounded drug products are not approved or evaluated for safety, effectiveness, or quality by the FDA.

**Please note, not all patients will qualify for a prescription and placing an order does NOT guarantee fulfillment of an order, until a consultation is obtained and medication is approved and prescription is sent to a partner compounding pharmacy by our medical provider. If the order is deemed unsafe, or unapproved due to other health/safety reasons, a refund will be issued immediately. Please note that Zeus Men’s Health has a right to deny issuing a prescription, or shipment of a medication from our partner pharmacy to a patient for health and safety reasons to the patient, or any reason deemed to be in any way harmful to the patient or the staff at Zeus Mens Health.

This agreement will be governed by and construed according to the laws of the State of California, without regard to its conflicts of law provisions. The state and federal courts in Los Angeles, California, will have exclusive jurisdiction for any disputes arising from this agreement not subject to arbitration.

8. Severability

If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions will continue in full force and effect. The invalid provision will be enforced to the maximum extent permissible to reflect the intent of the parties.

10. Acknowledgment and Consent

By using our services, you acknowledge that you have read and understood this Consent to Treat & Financial Responsibility Guarantee. You consent to the treatments and financial terms outlined above and confirm that you are agreeing to this document voluntarily.

*upon order receipt, the intake information will be reviewed by our medical staff, and a telehealth consultation will be arranged within 2
business days. There is no guarantee that the ordered medication will be approved by the medical provider until the consultation is held. If the medication is disapproved and deemed to be unsafe for the patient by the medical consultation, the patient will receive a full refund of their order amount. If the medication is approved and deemed appropriate and safe to continue with the order and subscription, a partner Pharmacy will be notified and will process the order based on their time schedule usually ranging from 2 to 5 business days. I understand this procedure and timeline is necessary to ensure the safe and proper delivery of medication to the patient in the most medically indicated manner.

Our partner pharmacies are also in compliance with all applicable laws and regulations related to the prescribing or dispensing of controlled substances, including all necessary, licensing, registration, authorizations required to prescribe and dispense controlled substances, as defined by the relevant jurisdiction, California. This also applies to the location/jurisdiction in which the patient is located, as well as the shipping address that the patient receives medications, also must all be within the state of California. We closely monitor any litigation regarding all of the medications and treatments we provide at Zeus Men’s Health. We strive to provide treatments with the highest standards, in full compliance with all local, state, and governmental laws and agencies, including the medical board, Pharmacy board, and FDA, under the jurisdiction of which Zeus Mens Health serves. We understand that these governing bodies maintain the right to limit our ability to prescribe or dispense any treatment or medication based on their rules, regulations and discretions. In the rare instance that this occurs, and we are no longer able to provide a treatment or medication due to a decision by a regulatory agency under which the jurisdiction of Zeus Men’s Health falls, we will make every attempt to provide our patients with an alternative treatment option. We cannot guarantee that the alternative treatment will 1) Be as effective as the discontinued treatment 2) Be as well tolerated as the discontinued treatment 3) Be of equal or comparable price as the discontinued treatment (often times the replacement may be significantly more expensive). By agreeing to the terms of this agreement, the patient understands these possibilities and risks of potential limitations of a treatment due to decisions made by regulatory agencies out of the control of Zeus Men’s health.
